Give Hope


Give Hope is a rapid social service navigation program that primarily focuses on homelessness/housing security, addressing mental health and substance use concerns, and providing client-led case management. Since 2019, Give Hope has conducted street outreach to individuals who are living in non-habitable locations, complete assessments on the individual and rapidly connect them to the services/agencies that can provide assistance. Give Hope meets individuals where they are to address their most basic needs. Conducting client-led case management allows clients to address concerns they may have and the assistance they may need/want.

Give Hope seeks to provide case management across a full continuum of services to achieve better health outcomes, reduce criminal activity, enhance community safety, decrease homelessness and vagrancy and increase livability and workability in “hot spot” areas. Community Catalyst has developed this model for pre-booking diversion, and with their permission, Volunteers of America has customized it for our approach to navigating individuals experiencing homelessness to needed services.

Give Hope has, through active participation in the Luzerne County Continuum of Care and collaboration with multiple social service agencies within the county, developed a broad housing, substance abuse, mental health and physical health network to support those experiencing homelessness.

In our pilot year, the program was funded by Pennsylvania Housing Home4Good, Northeastern Pennsylvania Health Care Foundation and Luzerne County.
