Community Health Services

Volunteers of America of Pennsylvania (VOAPA) has a growing team of community health professionals committed to addressing health equity and social determinants of health needs for individuals, families, and communities across Pennsylvania.

VOAPA’s community health workers and health care navigation services are available to many of our housing, children and family, and veteran programs. Our community health teams also work in community-based settings to be accessible and responsive to the pressing needs of the communities we serve.

Community health teams provide in-home visitation and on-site support to program participants and organize public community health events that offer education on chronic conditions, medication reviews, medical accessibility/affordability, referrals to early intervention services, and linkage to physical and behavioral health care providers.

Our community health services also address social determinants of health and offer direct service or referrals to individuals and families for childcare, housing, financial strain, food insecurity, clothing, utilities, employment, and transportation.
