VOAPA’s Community Health Services play a vital role in supporting the well‑being of individuals and families across our programs. Our community health workers and health‑care navigation teams provide direct support to participants in our housing, children and family, and veteran services, ensuring that people can access the care and resources they need to stay healthy and stable. By working in community‑based settings, our teams remain accessible, responsive, and deeply connected to the neighborhoods we serve.

Community health teams offer in‑home visitation, on‑site support, and public community health events designed to educate and empower participants. These events and visits may include information on managing chronic conditions, reviewing medications, improving medical accessibility and affordability, and connecting individuals to early‑intervention services. Staff also help participants navigate the health‑care system by linking them to physical and behavioral health providers who can address their long‑term needs.

In addition to health education, our community health services address the social determinants of health, recognizing that stability and wellness depend on more than medical care alone. We provide direct assistance or referrals for childcare, housing, financial strain, food insecurity, clothing, utilities, employment, and transportation. By supporting the whole person, VOAPA helps individuals and families build healthier, more resilient lives.
