Supportive Living Program

A program that serves adults with moderate to severe mental illness through the creation of self-directed recovery plans to maintain psychiatric stability and independence.

Program Eligibility:

Adults over the age of 18 who have been diagnosed with moderate to severe mental illness and live in Dauphin County. There is no income requirement.

Program Services:

  • Help identifying personal goals
  • Support strategies that encourage skills to reach desired goals
  • Linkages to supports to maintain personal goals
  • Navigation to understand mental health diagnoses and medications
  • Assistance identifying strengths, skills, abilities, and resources needed to recover
  • Development of a recovery plan to support goal attainment 
  • Training to use public transportation or shared ride applications

Program Referral Process:

Individuals must be referred through Dauphin County’s Case Management Unit, Keystone’s Intensive Case Management Unit or the Residential Treatment Team to receive services. VOAPA can also assist with the referral process to enroll individuals into its Supportive Living Program.
